Request medical records
Secure communication with your Northwest Eye staff and physicians

– Select “create an account”
– Complete the required fields
– The practice ID is UVPMRA and you will need your unique security code
*If your unique security code was emailed to you, it is only good for 72 hours
*If you do not know your unique security code, please call us at 612-355-6510

Don’t forget to bring:

Insurance card or proof of insurance
Picture I.D. with current address
Name of your current pharmacy, address and phone number
Co-payment or Deductible (payment is due at time of service)
